Defeating Xesht, We That Are One, the pinnacle Breach boss, offers a chance to acquire the Hand of Wisdom and Action. Xesht resides within the Twisted Domain, accessible via the Realmgate on the Atlas using a Breachstone.

Locate the Realmgate near the Atlas's starting point. Click the home icon on the Atlas to center the camera on this location.

Alternatively, the Path of Exile 2 official trading website offers HoWA, but at a substantial cost; even poorly rolled pairs command multiple Divine Orbs.

Acquiring Breachstones in PoE 2

Two methods exist for obtaining a Breachstone:

• Combine Breach Splinters: Farming Breach map nodes on the Atlas yields Breach Splinters. While relatively common, accumulating 300 requires extensive farming. Combine 300 splinters to create a Breachstone.
